Proceedings will be published as a volume of Progress of Theoretical Physics Supplement. Each speaker is kindly requested to send his contribution not later than *** Nov. 30, 2003 ***.

45 min. talks: 18 pages
40 min. talks: 14 pages
30 min. talks: 10 pages
25 min. talks: 8 pages
Poster : 4 pages

The files should be prepared using the PTPTeX format, and send the files to : fdqcd@riise.hiroshima-u.ac.jp
